ACP Practice Advisor® is a web-based tool to help clinicians, medical practice administrators, and other medical staff evaluate and make changes to the delivery of patient care. ACP Practice Advisor® consists of 3 groups of modules:
Building the Foundation
These modules address the key attributes and expectations of the Patient-Centered Medical Home (PCMH) model according to the major national certification organizations. This guidance is also applicable to all practices and specialties interested in providing value-based, patient-centered care or participating in Medicare advanced alternative payment models (APM's) such as Primary Care First, CPC+, or Accountable Care Organizations (ACO's). Course modules include: Communicate with Patients, Work as a Team, Improve Quality, Care Coordination, Improve Patient Access, Avoid Unnecessary Testing, Organize Your Practice, Deliver Patient-Centered Care, and Patient and Family Engagement.
Specialty Practice Recognition
These modules address the key attributes and expectations of specialty and sub-specialty Patient-Centered Specialty Practice recognition according to NCQA (2019 Edition). This guidance is also applicable to all specialty and sub-specialty practices and specialties interested in providing patient-centered care, even if not seeking qualification as a PCMH or PCSP. (NOTE: These modules do not offer CME or MOC).
Improving Clinical Care
These modules help practices apply the attributes of patient-centered care to improve the health of people and populations with specific conditions or clinical concerns. The information and resources are intended to improve care at the practice level, not to provide clinical education.

How it works:
Each ACP Practice Advisor® module is composed of several parts:

- ACP Practice Biopsy
- Background Material
- Case Study
- Resource Library
Modules offering CME and MOC practice assessment credit require that learners create a Practice Improvement Action Plan, and then come back after 30 days and take a follow-up Practice Biopsy in order to receive credit. Please follow the instructions at the end of the module to receive your CME credit and MOC points.
