ACP Practice Advisor® is a web-based tool to help clinicians, medical practice administrators, and other medical staff evaluate and make changes to the delivery of patient care. ACP Practice Advisor® consists of 3 groups of modules:

Building the Foundation 

These modules address the key attributes and expectations of the Patient-Centered Medical Home (PCMH) model according to the major national certification organizations. This guidance is also applicable to all practices and specialties interested in providing value-based, patient-centered care or participating in Medicare advanced alternative payment models (APM's) such as Primary Care First, CPC+, or Accountable Care Organizations (ACO's). Course modules include:  Communicate with Patients, Work as a Team, Improve Quality, Care Coordination, Improve Patient Access, Avoid Unnecessary Testing, Organize Your Practice, Deliver Patient-Centered Care, and Patient and Family Engagement.

Specialty Practice Recognition 

These modules address the key attributes and expectations of specialty and sub-specialty Patient-Centered Specialty Practice recognition according to NCQA (2019 Edition). This guidance is also applicable to all specialty and sub-specialty practices and specialties interested in providing patient-centered care, even if not seeking qualification as a PCMH or PCSP.  (NOTE: These modules do not offer CME or MOC).

Improving Clinical Care 

These modules help practices apply the attributes of patient-centered care to improve the health of people and populations with specific conditions or clinical concerns. The information and resources are intended to improve care at the practice level, not to provide clinical education.

Modules offering CME and MOC practice assessment credit require that learners create a Practice Improvement Action Plan, and then come back after 30 days and take a follow-up Practice Biopsy in order to receive credit.  Please follow the instructions at the end of the module to receive your CME credit and MOC points.

Banner Health Clinic - Phoenix, AZ

The clinic’s focus on standardization and continuous improvement—and its emphasis on patient-centered techniques to improve quality and reduce cost—has drawn the attention of the larger Banner Health System, which has used this clinic to pilot new initiatives, including efforts to streamline drug sample dispensing, implement a patient portal, and implement population management tools.
 
Baptist Medical Group: Memphis Primary Care - Memphis, TN

Baptist Medical Group: Memphis Primary Care is noted for seeking and responding to feedback and using it for improvement in the patient experience.  The practice staff systematically ensures that patients get recommended care, whether in or out of the practice, through careful follow-up.  Additionally, physicians have a strong commitment to keeping patients out of the hospital by providing robust patient education and engaging in shared decision making.
 
Family Physicians Group - Kissimmee, FL

Family Physicians Group, Kissimmee prides itself on being available to patients 24/7 and has home monitoring in place for their highest risk patients.  Case managers lead care planning for complex patients and a “concierge” is available to help with potential obstacles like transportation and translation for their largely Spanish-speaking, socioeconomically disadvantaged patient population.  Staff members maintain close relationships with a select group of specialists and track the quality and affordability of these specialists’ care to ensure value.
 
Ridge Wood Med-Peds: Rochester, NY

Ridgewood’s physicians share a passion for practicing medicine using evidence-based principles.  The physicians follow evidence-based protocols that enable them to delegate many tasks to other members of their care team.  Patients with chronic illnesses are prompted for follow-up visits and tests by indexing those visits to prescription refills.  Physicians refer to a small, informal network of trusted specialists, and carefully manage the referral process.
 
South Cove Community Health Center - Quincy, MA

South Cove Community Health Center maximizes patient access to care.  The center is open seven days a week, accommodates patient walk-ins, and offers a 24-hour nurse advice line.  Referrals are carefully coordinated, often include a translator, and are stratified for urgency.  Mental health is integrated in a culturally appropriate way, and patients receive support to help them maintain health insurance coverage.
 
SureCare Medical Center - Springboro, OH

The team at SureCare has improved access and extended hours while keeping team morale high.  Staggering schedules are one way that the clinic maintains a good work-life balance across the care team. SureCare has cross-trained MAs and standardized workflows to increase office efficiency and patient flow.  Physicians refer patients to each other initially, leveraging expertise developed in dermatology and orthopedics.
 
Cascade Internal Medicine Specialists - Bend, OR

Cascade Internal Medicine Specialists is a Medicare CPC+ Track 1 practice.  The staff is providing high quality, value-based patient care by doing simple tasks such as utilizing pre-visit planning forms before the arrival of each patient.  MA's are also performing at their highest scope of practice with the use of standing orders for procedures such as immunizations and EKG's.  The clinic has implemented EHR interoperability with local hospitals as well as with the state health information exchange in order to streamline patient care and reduce unnecessary testing/procedures.  Finally, the clinic has also been nationally recognized as a hypertension control champion through the CDC's Million Hearts campaign.
 
Transforming Clinical Practice Initiative

The Transforming Clinical Practice Initiative (TCPi) is designed to help clinicians achieve large-scale health transformation. The initiative is designed to support more than 140,000 clinician practices over the next four years in sharing, adapting and further developing their comprehensive quality improvement strategies. The initiative is one part of a strategy advanced by the Affordable Care Act to strengthen the quality of patient care and spend health care dollars more wisely. It aligns with the criteria for innovative models set forth in the Affordable Care Act:

  • Promoting broad payment and practice reform in primary care and specialty care,
  • Promoting care coordination between providers of services and suppliers,
  • Establishing community-based health teams to support chronic care management, and
  • Promoting improved quality and reduced cost by developing a collaborative of institutions that support practice transformation.

ACP Practice Advisor has been used by thousands of clinician practices within TCPi since the initiative began in September 2015.
