Quick Start: Patient Centered Medical Home

Why is This Important?

This module is intended to give practices a jump start towards applying for Patient Centered Medical Home (PCMH) recognition using the 2017 PCMH standards. The 10 important elements listed in this module are listed across national PCMH recognition programs including the National Committee for Quality Assurance (NCQA), the Utilization Review Accreditation Commission (URAC), and the Joint Commission.  Use this module to identify possible challenges early in your transition to the PCMH model. Then, at a pace you and your practice team determine, work through all of the Building the Foundation modules to get a complete sense of the opportunities for improvement.

How will these tools help?

Quick Start: PCMH is not a typical ACP Practice Advisor module. Use this module to help your practice team gain a basic understanding of the work needed to become a PCMH. Practices should complete all of the Building the Foundation modules since this Quick Start: PCMH module features just a small subset of the attributes associated with a well-functioning medical practice.
