Thank you for subscribing! ACP Practice Advisor® is a web-based practice improvement tool to help clinicians, medical practice administrators, and other medical staff evaluate and make changes to the delivery of patient care in ambulatory and inpatient settings. This is a subscription product.

ACP Practice Advisor® consists of 3 groups of modules:
These modules address the key attributes and expectations of a practice with a particular focus on what is needed to become a Patient-Centered Medical Home (PCMH) model according to the major national certification organizations. This guidance is also applicable to all primary care and specialty practices interested in providing value-based, patient-centered care or participating in Medicare advanced alternative payment models (APM's) such as Primary Care First, CPC+, or Accountable Care Organizations (ACO's).
These modules address the key attributes and expectations of specialty and sub-specialty Patient-Centered Specialty Practice recognition according to NCQA (2019 Edition). This guidance is also applicable to all specialty and sub-specialty practices and specialties interested in providing patient-centered care, even if not seeking qualification as a PCMH or PCSP. (NOTE: These modules do not offer CME or MOC.)
These modules help practices apply the attributes of patient-centered care to improve the health of people and populations with specific conditions or clinical concerns. The information and resources are intended to improve care at the practice level, not to provide clinical education.
The following modules offer practice assessment CME and MOC credit:

- Hypertension: Diagnosis and Initial Treatment
- Hypertension: Management of Uncontrolled Hypertension
- Managing Diabetes Mellitus
- Opioid Risk Management
- Stroke Prevention in Atrial Fibrillation
Each ACP Practice Advisor® module is composed of several parts:
- ACP Practice Biopsy
- Background Material
- Case Study
- Resource Library
Modules offering Practice Assessment CME and MOC credit require that learners create an action plan, and then come back after 30 days and take a follow-up Practice Biopsy in order to receive credit. Please follow the instructions at the end of the module to receive your CME credit and MOC points.

ACP Practice Advisor® is a subscription based resource, with a discount for ACP members. If you are having any issues logging into your account, please email us at firstname.lastname@example.org.
Credit is only available at the individual module level.
ACP Practice Advisor® is a subscription product. If you would like to access all modules for one full year, click here. Once you have registered, you will receive an email confirming your subscription. If you have completed the purchase, click "Enroll" (at the bottom of the page) to access all Practice Advisor modules. Once you have enrolled, you will be automatically enrolled in all courses and will then be able to take each individual course.
